GED Enrollment

HOW TO SUBMIT DOCUMENTS:
The process for enrollment is listed below and must be completed before classes begin. Please scan and
upload documents in PDF format and send via email to: GED@sanjosecyber.org. We request all
documents to be submitted electronically. Please do not take pictures of documents and submit them as
uploaded images. All documents submitted electronically must be in PDF format. If need be, you may fax
documents to our corporate headquarters located in Jacksonville, Florida at: (904) 569-0006. If needing
to provide a physical copy of a document by mail, please submit to:

  1. Email GED@sanjosecyber.org to let us know you are enrolling in the GED Program. Please
    include the student name in the email.
  2. New Student Registration = Required; complete online at
    https://e2020.geniussis.com/PublicStudentSignUp.aspx?did=3369
    - Fill out the information and press save
    - Do not pick out courses, we will do this for you
    - The form for homeschool does not work on the website, please see #3 below
  3. Submit Notice of Intent to Establish a Home Education Program; Required; can be found at
    https://dcps.duvalschools.org/cms/lib/FL01903657/Centricity/Domain/12537/Notice%20of%20Intent%20to%20Establish%20A%20Home%20Education%20Program%202021__ADA%20Compliant.pdf. Please send it back to admissions@sanjosecyber.org. We will send it to the homeschool
    office for you.
  4. Birth Certificate = Required; send as PDF document to GED@sanjosecyber.org
  5. Picture ID = Required; send as PDF document to GED@sanjosecyber.org
  6. Proof of Residency (Lease, utility bill, mortgage, bank statement, State ID) = Required; send as PDF
    document to GED@sanjosecyber.org
